We have the necessary medical and health care professionals right here at our disposal. We have doctors, nurses, pharmacists, biologists and other scientists right here at Inkundla. Now instead of us wasting their talents by utilising them to define such useless (but useful) issues like "umtshaza" , "inkophe", "inzipho", "amasendlisi" "umsundulo" and the like, I hereby suggest that we have an online health-care facility where we can field questions to them relating to serious medical and health problems that our people encounter daily.

For a start, we have Mthwakazians in the rural areas who suffer from all manner of illnesses, from head aches to diarrhea to malnutrition to dingindi to kwarshiokor to drops to syphilis to gonorrhea to mhedehede etc etc.

Now my proposal is that whosoever has a health problem (or his family back home), can ask our professionals like Dokotela , Mabila and others to answer and advise on such issues. What medication to buy, what diet to eat for those with obesity problems, what exercises to do for fitness enthusiats, what to do when you get infected with drop, what about bilharzia or malaria, what do you do when someone is suffering from meningitis, what about the endless illnesses that affect our kids.
One must emphasise that the information given here will be given in good faith and whosoever uses it does so at their own risk.

To kick start the whole process, let me set the ball rolling by asking our health professionals to give us a typical Mthwakazian diet which we can follow to stay health, no cholestrol, fat etc. What exercises to follow for those of us who spend too much time in the office with very little physical activity.

I will make efforts to answer your questions clearly and as simple as posible. I wish this provision not to be abused by anyone of us, but be used to benefit the Mthwakazi people.

Because this thread is for our healthcare issues, I will be very brief in my answer to your concern above. Firstly I acknowledge and respect your views relating to "izitho zomzimba" and their description in Ndebele. One must also further acknowledge that as a medical and healthcare professional discussing these features is just part and parcel of your profession hence acceptable. But my feeling is that culturally, our sociatal norms have taught us that these issues be "respected" and be regarded as taboo particularly for younger persons. Taboo in the sense that our youth must never engage in any activity that combines these two poles and generate heat in the process leading to loss of libido (idobi?) nk nk.
Iam merely emphasising the cultural aspect to this issue.
because we are now educated does not mean we should forego or discard our cultural value systems. Look at the Middle East, because of their strong laws prohibiting promiscuity and adultry (ukweqa uthango) which carries a possible death sentence, HIV-AIDS is virtually unknown there.
Now, because through education we learn that in Korea for example they eat dogs and cats as a delicacy, it does not necessarily mean we must now have a cartblanche wanton slaughter of our pets and start braaing them. The Japanese have embraced education but maintained their culture. The Chinese have done the same. We learn from tales that in the DRC the Kasayis eat fellow human beings, that does not mean all of a sudden Lobengula must eat Mabila!!!!! nk nk nk. Not that I can't eat you( look at my picture >>lion!!!), no, but i must not do so only just because I have learnt this from "education". I must preserve my culture.

Further diet tips:

Replace all FANTA drinks with plain water. Carbonated drinks have no food value. Kuhle khona ukunatha up to 2L of water ngelanga. Hatshi i beer, plain manzi.

Eat at least 3 fruits ngelanga and keep the doctor away. Nanka awile ama orange eGwana. Nice.

Eat lots of vegetables carrots, beets, cucumber, tshomoliya, cabbage, brocolli, mathanga. Umganu wakho kawube colourful with greens, yellows, whites etc uyabe usudla kuhle ngempela.

Lingabi legunya bo Baba nxa liphiwa imiganu yokudla eqongileyo lithi yikho ukuba ngubaba womuzi, kulesifo lokhuya. Dlanini okulingeneyo and even demand smaller size plates.

Ukuhlafuna controls the amount of food we eat. Chew everything thoroughly loba kumnandi. Ingqondo gets the message that isihlafune kakhulu and sends a feedback message yokusutha.

Nxa usizwa angani awusuthanga, ungagijimeli ukwengezelela khonokho khonokho ukuthi usuthe uthi nsti, give yourself 30 minutes. If you are still hungry then, eat some more.

No major problems with musturbation. You must not do it excessively. Recent discoveries show that too much sex or musturbation reduces your sperm-count thereby reducing probability of making a fertile woman pregnant.

Because sperms have alife span of 3 days it is recommended to have about 3-4 ejaculations per week to maximise chances of fertilisation. Therefore musturbation may not be viewed as a health threat, if done properly.

If musturbation causes sterility, we would not all have been born. Your body reduces the sperm during your sleep.

I will give more technical details, if need be.

Many things may cause iqolo ukuthi libe buhlungu. One othe cammon causes that i have seen is that of Repetetive Stran Injury (RSI), caused by improper use of gadgets like chairs and desks in a working environment or at home.

This form of injury can best be avoided by taking proper precaution in using chairs (proper sitting), desks and other things which they use predominantly.

Symptoms are severe and may grow if the person does not improve their behaviour. Common examples of RSI are the Carpal tunnel syndrome (CTS), which affects mostly typists.

Judging by the mystery and mysticism associated with this "ngubhane" it seems there is more to it than meet the eye.
The answers given by our very own Mabila, Dok and others above suggest that we still have to learn a lot about this strange disease called "ngubhane". The question which we want to put to our esteemed medical team is: What exactly is this "ngubhane"? Is it a euphemism for something much bigger or its just one of those taboo subjects?
When one takes Laxative tablets in order to clean one's tummy or stomach , is this not part and parcel of cleaning the body of this dreaded but mysterious "ngubhane"?.

That was on Saturday deep into the night because there was a Miss Big pageant at Windermere so all Mthwakazians were there in full force with izimomondiya zabo on the side and beer in hand. Ok. i will try and explain my understanding of ingubhane and the peyiding part. As i have said above that i have not inspected the active substance in intolwane which acts as an astrigent in colonic irrigation. There are many oter herbs which have the same effect though.

You will notice that "Ingubhane", is a symptom not a disease. It is characterised by flatulence, colon distress, diverticulosis.

Uku peyida (colonic irrigations), is a therapy that helps to release the tension in the colon, thereby improving metabolic eficiency. Intolwane acts as an astringent for people with ingubhane, it cleans the walls of the intestines. We will also notice that when a person has such symptoms, they are normally dehydrated as well because of constipation, therefore taking beer or other deuritics is not recommended.

Weakness, headaches that you feel during the "ngubhane" period is a sign of dehydration. I recommend plenty of water, veggies, fruits and some herbal astrigent that may be of help (intolwane).

The whole process of ukupeyida, inproves the absorption of certain vital body nutrients.

Bakwethu! Inyongo as we call it esindebeleni, is characterized by headaches, stomach aches, nausea, dizziness, dificulty in swallowing, belching and numbness and eventually emesis (vomiting).

Inyongo, in simple terms is an effect which is commonly experienced from backflow of stomach acids. Mild cases may be cured by some anti-acids, vomiting may relieve it.

It is very important to be diagonised because cases of gastroenteritis are also common and have some similar symptoms may need antibiotics. Premenstrual syndrome may also cause nausea.

Alcohol, sugary food, tends to raise the stomach acids, pancreatic and intestinal secretions. Natural herbs may also be helpful for nausea, e.g peppermint tea.

With malaria the symptoms r quite severe i would thought, u do get same symptoms as inyongo like nausea, vomiting and abdominal cramps and headache,but in addition to that u get high fever, sweating, uqhuqho, and joint pain.
What i would like to know from the experts is how do u get cerebral malaria?

Lungs lose power at lunch time

Human lungs experience a dip in performance around noon and not just at dawn as previously thought, suggests a large study into the effect of circadian rhythms on lung function.

The findings suggest doctors could lower drug doses to respiratory patients by administering the drugs shortly before noon, when most needed.

"Almost every single physiological process follows certain circadian rhythms," says study author Boris Medarov, a physician at the Long Island Jewish Medical Center in New Hyde Park, New York, US. Sleep is the most obvious example of a process following a regular, 24-hour cycle, but hormone levels controlling growth, metabolism and the immune system follow a daily pattern too.

Various health problems also tend to peak at certain times of day. Heart attacks strike most often in the morning, while night-time sees the most asthma attacks.

Studies of asthma patients show the worst symptoms are most likely between 0400 and 0600, before improving steadily during the day. Symptoms are least evident around 1700 before intensifying again at night.


Lunch-time dip


Medarov studied the lung capacity of about 4800 patients, plotting each measurement against the time at which the test was taken, between 0800 and 1700. He found that lung function actually dips at noon before peaking in the late afternoon.

His results show a 15% to 20% difference in lung performance between noon and late afternoon. However, healthy people have such strong lungs that such a change would hardly be noticeable, he says.

"That difference would basically apply to people with lung disease - asthma, chronic bronchitis or emphysema," he told New Scientist. "Considering this information, they may better time their medication, which theoretically may lead to less use. Also, the late afternoon may be the best time for those people in rehab programs to take exercise."

Medarov presents his study on Wednesday at a meeting of the American College of Chest Physicians in Seattle, Washington. He admits that he cannot disprove previous studies that show lung function is worst around dawn, since his study was limited to working hours.

And his research made no allowance for health of its participants, so its applicability to people with asthma or other specific disorders is unknown.


Case-by-case basis


"If for some reason the healthy people came at 5pm and the sick people came in the morning, that would fit this result," says Nizar Jarjour, a pulmonary specialist at the University of Wisconsin in Madison, US.

"But the idea that lung function does change during the daytime - that's interesting," Jarjour told New Scientist. He says changing patients' medication times can indeed affect both a medicine's effectiveness and its side effects.

But he adds that doctors must consider the timing of drugs on a case-by-case basis, making sure asthmatics' doses cover night-time, when previous studies have shown attacks peak.

Jarjour says hormones are probably responsible for the lungs' circadian rhythms, though when people lie down at night, the horizontal orientation could also inhibit lung function. "But the very interesting unanswered question is why human beings were built to have better lung function in the afternoon hours," he says.

Nose bleeds, are caused by a number of reasons which include:

1. Genetical influence, inherited from parent/s.
2. Diet, Just as you say, could cause quiCker chances of nose bleed. WheN certain nutrients required to make you blood clot, you may experience nose bleeds easily and if its women they may experience a heavier menstrual flow.
3. Pressure and/or injury on blood vessels on the nasal septum. This normally happens to those having a flu and have running nose, which seems blocked and they make frequent blows.

4. Use of asprin amay cause it as well.

Firstly, i would like to warn all those those that have a tendency of googling medical information from the internet. Google is the worst thing that you can ever do when you need medical publications, because it contains a lot of useless junk which is normally dumped for commercial reasons. About 95% of the stuff you will find on the internet is rubbish.

Mhlanga, for constipation i suggest drinking lots of fluids, mainly non-deuritics like water and fruit juices. Studies have shown that the menstruating women tend to have a looser bowel. Age is also another factor and collected data suggests that the changes in the bowel habits of women of climacteric age at menstruation are greater for those suffering from constipation than those who are not.

Harare - Zimbabwe's HIV prevalence rate has dropped by 4,5 percent in the past two years because of changed sexual behaviour, a state-run daily reported on Monday, quoting from a recent survey.

"The percentage of Zimbabweans between the ages of 15 and 49 infected with HIV has dropped from 24,6 percent to a still very high 20,1 percent in the last two years," The Herald newspaper said.

"This makes Zimbabwe the second country in sub-Saharan Africa, after Uganda, to see its HIV infection rate start dropping."

Health and Child Welfare Minister David Parirenyatwa said the survey by Zimbabwe's health ministry, the UNAids agency and the US Centre for Disease Control also showed a decline in sexually transmitted infections.

"Everyone now seems to know the importance of preventing HIV and to an extent are trying their best to avoid getting infected, which should mean they are practising safe sex," Parirenyatwa told the newspaper.

"Girls are now delaying when it comes to starting sexual activity and almost everyone in the country has an understanding of what HIV is all about."

Zimbabwe is one of the countries hardest hit by the HIV and Aids pandemic with at least 3 000 people dying weekly from Aids-related illness - or about one person every three minutes - according to the National Aids Council.

Zimbabwe's battle with the pandemic has been compromised by a collapsing public health sector and dwindling donor funding due to strained relations between Harare and its former Western benefactors.

The government collects a monthly levy from workers to fund HIV and Aids projects.
